Listed below are forms which need to completed and returned to the Residence Life Office.
There are two ways to submit the forms:

  • Download the PDFs, complete the forms in black or blue ink and fax them to 001-740-376-4420.
  • Download the Microsoft Word Documents, complete the forms using Word and email them to seel@marietta.edu. Or print the files and fax them to 001-740-376-4420.

Be sure to make a copy for your records.

Meningococcal and Hepatitis B Vaccinations

The 125th General Assembly of the State of Ohio passed House Bill 142 which, in summary, says:

Students living in college dormitories will need to either be vaccinated for Meningitis and Hepatitis B or sign a waiver stating they understand their increased risk but do not wish to have the vaccines.
Students are responsible for either getting the vaccines or completing the waiver form prior to enrolling at Marietta College. (The Student Health Center does not offer the vaccines, but the Residence Life Staff strongly encourages incoming students to consider getting vaccinated.)
