Marietta College student-athletes are annually required to fill out and return an insurance form and sports medicine athlete history form before they can participate in any athletic-related activity.

In addition, first-time student-athletes are also required to fill out the physical form.

Student-athletes wishing to discuss a possible transfer to another NCAA D-III school must first fill out and submit a self-release form.
